The song is (originally) about drug abuse and personal pain. It is about the damage he (Trent) did while he was an addict, to himself and those around him. In Cash's context I think its probably safe to change drugs for selfishness and booze, like most true talents part of Cash's genius came from his flaws. The amazing thing is how this can have so many different meaning for such a wide audience and how a song written with such focussed & personal meaning for Reznor can be transformed by Cash

Every time I watch this video - all I can think at the end is - WOW. I'm almost 50 myself and have listened to Johhny Cash for many, many years - and also love Nine Inch Nails - but he just owns this song. 14 million plus views says that he nailed it!
